All medications, even over-the-counter (OTC) and supplements MUST be brought to Nurse Benjamin.

FOR LONG TERM MEDICATION (LONGER THAN 2 WEEKS) IN SCHOOL:

MED-1 / Request for Administration of Medication, Treatments or Use of equipment in School

It must be signed by both the doctor and the parent. This is required EVEN for OTC medications. The medication itself must be properly labeled by the pharmacy with all instructions and student name (if prescription) intact.

S_865 / Student Health Status

It provides emergency information for Nurse Benjamin and a place to develop an individual health plan for the student.

These forms MUST be renewed EVERY school year, even if the treatment remains the same.

SHORT TERM MEDICATIONS:

A medication, such as an antibiotic needed to be administered four time daily, must be given to Nurse Benjamin in a pharmacy-labeled container.
