Note from Nurse Benjamin

Dear parents,

The Oral Health Impact Project (OHIP) (the dentists!) will be here at McCall through most of this week. If you have already sent in a consent form for your child to be seen, there is no need for you to do anything. If you don’t think you completed a consent form and would like your child to be seen by the dentist at school, please contact me. If you think you completed a form and DO NOT want your child to see the dentist, please let me know that as well. OHIP will bill your insurance if you have dental coverage.
Does your child have medication stored in the Health Room? We are nearing the end of the year. Please contact me to make a plan to send the medication home for the summer. I will also provide the forms you need to bring the medication back next year. I am also attaching some key health forms (MED1, MEH-1, MEH-155) to this message.
Thank you for another fantastic year with the most wonderful children in Philadelphia! It is a pleasure to care for them!
Have a wonderful summer!
Nurse Benjamin
